29 CFR  1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:  a) Jobsite located at 3731 Lancaster Avenue, Philadelphia, PA - The employer did not ensure that work areas and passageways throughout the job site were kept clear of scrap lumber, protruding nails and other debris; thus exposing the employees to impalement, trip and fall hazards.  Condition was observed on or about November 1, 2021.

29 CFR 1926.501(b)(4): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a) Jobsite located at 3731 Lancaster Avenue, Philadelphia, PA - The employer did not ensure that a form of fall protection system such as but not limited to a guardrail, was provided for employees working next to an open floor hole, exposing them to fall hazard of approximately 8 feet to the floor below.  Condition was observed on or about November 1, 2021.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with this part.  a) Jobsite located at 3731 Lancaster Avenue, Philadelphia, PA - The employer did not develop and implement an accident prevention program to assist in identifying working conditions which are hazardous to the health or safety of his employees, such as, but not limited to, fall hazards.  Condition noted on or about November 1, 2021.  RLC Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA inspection number 1440532, citation number 1, item number 1 and was affirmed as a final order on 5-15-2020, with respect to a workplace located at 625 W Girard Ave, Philadelphia, PA.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Jobsite located at 3731 Lancaster Avenue, Philadelphia, PA - The employer did not ensure that employees conducting demolition activities and working on the 2nd floor of a residential building in proximity to open and unprotected sides and edges, were provided with fall protection system. Employees were exposed to fall hazards of approximately 12 feet.  Condition was observed on or about November 1, 2021.  RLC Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1440532, citation number 2, item number 1 and was affirmed as a final order on 5-15-2020, with respect to a workplace located at 625 W Girard Ave, Philadelphia, PA.
