The rotating chuck on the the drill press in the machine shop was not guarded, exposing employees to injury from rotating parts of equipment.

29 CFR 1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Employees were exposed to struck-by hazards. Employees using the bench grinder in the assembly shop to sharpen tools were exposed to struck by hazards in that the work rest was approximately 3/4" away from the grinding wheel.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: Employees were exposed to amputation and struck-by hazards. On or about November 16, 2021 employees were using a bench grinder to sharpen tools in that the tongue guards were missing on both sides of the grinders

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws. Employees were exposed to electrical shock hazards. Employees in the welding shop were exposed to electrical shock hazards while using the Dewalt angle grinder. The outer cord insulation had pulled away from the plug therefore strain relief was not provided.
