29 CFR 1910.147(f)(1):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions required by this paragraph were not followed: a) Production floor, Hess Machine RH2000 - On or about November 15, 2021, when trouble shooting the machine required it to be energized, employees were not removed from the danger zone before it was re-energized.
