This condition exposed employees to cave-in and struck-by hazards associated with excavation wall collapse.  (b)H & W Contracting, L.L.C. at Main Street & Washington Avenue, Salem, SD 57058:  Between November 16, 2021 and November 22, 2021 and at times prior to, the controlling and exposing employer did not ensure that employees and subcontractor's employees were protected from cave-in hazards while working within the southeast corner excavation (excavation number 4) that measured greater than seven feet in depth.  This condition exposed employees to cave-in and struck-by hazards associated with excavation wall collapse.  (c)H & W Contracting, L.L.C. at Main Street & Washington Avenue, Salem, SD 57058:  On or about and at times prior to November 15, 2021, the employer did not ensure that employees were protected from cave-in hazards while working within the southeast central excavation (excavation number 3) that measured greater than nine feet in depth.  This condition exposed employees to cave-in and struck-by hazards associated with excavation wall collapse.  (d)H & W Contracting, L.L.C. at Main Street & Washington Avenue, Salem, SD 57058:  On or about and at times prior to November 10, 2021, the employer did not ensure that employees were protected from cave-in hazards while working within the southwest central excavation (excavation number 2) that measured greater than eight feet in depth.  This condition exposed employees to cave-in and struck-by hazards associated with excavation wall collapse.
