Colorado Springs, CO 80909: On or before 11/30/2021 the employer did not protect employees from injury in that the Lodge and Shipley shear [model: 20T24, serial#: 47063] was not guarded in accordance with this standard. The guard had an approximately 1/2 inch opening and the size stock for this machine was thin gauge sheet metal. The operator's fingers went under the guard while running pieces through the shear. A mechanical clamp preceded the shear blade behind the guard. This condition exposed employees to laceration and amputation hazards.   (b) Metal Products, LLC at 2717 Willamette Pl. Colorado Springs, CO 80909: On or before 11/30/2021 the employer did not protect employees from injury in that the custom-made step flashing shear was not guarded in accordance with this standard. This machine was used to make step flashing products. The machine was equipped with a guard that moved along with the shear blade and the arms moving the guard were not guarded. This condition exposed employees to laceration and amputation hazards.  (c) Metal Products, LLC at 2717 Willamette Pl. Colorado Springs, CO 80909: On or before 11/30/2021 the employer did not protect employees from injury in that the Minister Machine Co. electric shear brake combo [model: OBI No. 6, serial#: 15028] was not guarded in accordance with this standard. The machine ran coiled sheet metal through a die where it was cut to length and bent. The machine was not equipped with 2-hand control or a guard. The front and sides of the die were exposed. This condition exposed employees to laceration and amputation hazards.  (d) Metal Products, LLC at 2717 Willamette Pl. Colorado Springs, CO 80909: On or before 11/30/2021 the employer did not protect employees from injury in that the Roper Whitney auto brake [model: AB1074T2, serial#: 1410909] was not guarded in accordance with this standard. This machine was used to make drip edge and flashing products. For bends involving the drip edge, the operator's hands came within 4 inches of the jaws. The auto brake was not equipped with guards or presence sensing devices to protect the employees. This condition exposed employees to laceration and amputation hazards.  (e) Metal Products, LLC at 2717 Willamette Pl. Colorado Springs, CO 80909: On or before 11/30/2021 the employer did not protect employees from injury in that the Roper Whitney auto brake [model: AB1074T2, serial#: 1220608] was not guarded in accordance with this standard.
