Employees did not apply energy controls by; turning off and/or shutting down the hydraulic motor, isolating the air and shutting down the electric drive motor and gear box [per the 1910.147(d)(2) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to the energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].
