29 CFR 1910.27(b)(2)(v): The employer did not ensure that the rope descent system has proper rigging, including anchorages and tiebacks, with particular emphasis on providing tiebacks when counterweights, cornice hooks, or similar non-permanent anchorages are used. Location: 85 Jay Street, Brooklyn, NY. On or about 12/10/2021 a) The employer did not ensure the rope used for each employee's boatswain's chair was secured to an anchorage and tieback. An employee in a boatswain's chair fell approximately 190 feet when the rope was not secured to any anchorage.

An employee using a boatswain's chair fell when the rigging of the system was not installed properly.
