Location: 175 Greenwich St. New York, NY. On or about 11/16/2025, a) The employer did not inspect employees' walking-workings surfaces for safety hazards prior beginning work. Employees worked from a glass canopy with unprotected edges and approximately 12 feet above the lower levels.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling. Location: 175 Greenwich St. New York, NY. On or about 11/16/2025, a) Two employees worked from atop a canopy and did not utilize any fall protection. An employee fell approximately 12 feet from an unprotected edge of the canopy while cleaning the glass panes above it.
