Washington St. Van Wert OH 45891, did not erect toeboards, screens or guardrail systems to prevent objects from falling 14 feet 4 inches from higher levels. The employer did not erect a toeboard on the second level wall opening of 117 N. Washington St. Van Wert OH 45891 during an active commercial demolition site where six employees were engaged in commercial demolition which involved removing debris and working on both the second level, and the ground level exterior where debris was being thrown into commercial dumpsters.

The employer did not protect employees from falling 14 feet 4 inches to the ground below, through a 12 inch X 11 inch hole in the walking working surface on the second level of a commercial demolition site.
