29 CFR 1926.62(d)(1)(i):Each employer who had a workplace or operation covered by 29 CFR 1926.62 did not initially determine if any employee was exposed to lead at or above the action level of 30 micrograms per cubic meter of air (30 �?g/m3) calculated as an 8-hour time-weighted average (TWA): a) Aztec Services Group, Inc. at the jobsite located at 101 East Main Street in Van Wert, Ohio: On or about December 21, 2021 the employer did not conduct an assessment to determine if employees were exposed to lead at or above the action level. Employees were exposed to hazards associated with lead exposure while performing punch list items such as but not limited to, pull nails out of floor boards. A bulk dust sample collected of dust and debris found on the floor in the area the employees were working revealed to be composed of approximately 0.78% lead.

29 CFR 1926.1127(d)(1)(i): The employer did not establish the applicability of this standard by determining whether cadmium was present in the work place and whether there was the possibility that employee exposure will be at or above the action level prior to performance of any construction work: Aztec Services Group, Inc. at the jobsite located at 101 East Main Street in Van Wert, Ohio: On or about December 21, 2021 the employer did not determine whether cadmium was present in the workplace and whether there was a possibility that employees exposures would be at or above the action level. Employees were exposed to hazards associated with cadmium exposure while performing punch list items such as but not limited to, pull nails out of floor boards. A bulk dust sample collected of dust and debris found on the floor in the area the employees were working revealed to be composed of approximately 0.0024% cadmium.

A bulk dust sample collected of dust and debris found on the floor in the area the employees were working revealed to be composed of approximately 8% silica quartz.

Employees were exposed to hazards associated with lead exposure when eating and drinking while taking breaks in the building located at 107 East Main Street in Van Wert, Ohio. Wipe sample analysis found lead contamination, 12.232 �g, on a cooler lid found where employees take their breaks.
