29 CFR  1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.   (a): Interior Second Floor -  Employees were using a folding A-frame ladder on top of the platform of a mobile scaffold. Employees were using a folding A-frame ladder to  increase their working height from the mobile scaffold, on or about December 13, 2021.   There is no abatement certification or documentation required for this item.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a): Interior Second Floor -  Employees were not trained on scaffold use. Employees were painting the ceiling using mobile scaffolds. Employees were using a folding ladder on top of a scaffold, on or about December 13, 2021.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were painting the ceiling using interior paint, primer and caulk. Hazardous chemicals included in those products included, but were not limited to, Titanium Dioxide, Limestone and Silica Amorphous on or about December 13, 2021.   Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
