29 CFR 1910.28(b)(2)(i): The employer did not ensure that each employee in a hoist area is protected from falling 4 feet (1.2 m) or more to a lower level by one of the systems listed in 29 CFR 1910.28(b)(2)(i)(A)-(C).  Warehouse:  Employees perform material handling operations on the mezzanine level by retrieving items hoisted up by a forklift.  Employees open a swing gate, creating an open-sided edge, which exposes them to fall hazards approximately 11 feet above the lower level.

29 CFR  1910.213(h)(4):Radial saw(s) were not installed in a manner so as to cause the cutting head to return gently to the starting position when released by the operator:  Wood Shop:   Employees used the Delta radial arm saw to crosscut wood for tables, podiums, and other furniture.  When released by the operator, the saw blade remained in its location without returning to a home position.  Employees were exposed to amputation hazards.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  In the following locations, relocatable power taps, not approved for the locations and use, provided power to electrical equipment.  a) Upholstery Shop.  A relocatable power tap was used to energize a Panasonic microwave and Avanti refrigerator.   b) Basement Shop.  A relocatable power tap was used to energize an Amana commercial microwave and Avanti platinum refrigerator.  Employees were exposed to electrical and fire-related hazards.
