29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    (a) Roof - Employees were exposed to fall hazards in that they did not use full body harnesses that distributes the fall arrest forces over at least the thighs, pelvis, waist, chest and shoulder while working on a residential roof of approximately thirty (30) feet, on or about December 16, 2021. 29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Roof - Employees accessing a residential roof were exposed to a fall of approximately twenty-one (21) feet in that the fiberglass extension ladder used was not long enough to allow the ladder side rails to extend at least three (3) feet above the upper lander surface, on or about December 16, 2021.  Abatement Certification Required.
