29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not ensure lockout/tagout procedures were implemented when an employee was required to service the Accurpress CNC hydraulic Brake Press (S/N 8182-445429/17) to include placing a part of his body into an electric power panel on a machine in the fabrication area. This condition exposed employees in the fabrication area to amputation and electric shock hazards. (b)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not establish a program consisting of machine specific energy control procedures, including regular inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative. This condition exposed employees in the fabrication area to amputation and electric shock hazards.

This condition exposed the powered industrial truck operator to crushing hazards.

29 CFR 1910.179(j)(2)(iv):Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: (a)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not ensure that monthly inspections of hoist chains, with signed reports, were performed. Employees used multiple cranes and hoists, including multiple 3 ton R&M overhead cranes and a 2 ton CM hoist, to transport fabricated steel components in and about the production floor. The employer did not ensure that the monthly inspections of hoist chains and ropes were documented. Three lifting chain hooks in use were identified with no safety latches in place or had been removed. This condition exposed employees to a struck by hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not ensure that the unused portion of blade on the vertical bandsaw, Hem saw - Vertical Bandsaw S/N: 1172513 was properly adjusted with a guard or shield. Approximately seven inches of saw blade was exposed while cutting rectangular bar stock. This condition exposed employees to amputation hazards. (b)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not ensure that the unused portion of blade on the horizontal bandsaw, Hem saw - Horizontal Bandsaw S/N: 1280278 was properly guarded with a guard or shield. Approximately eight inches of saw blade was exposed while cutting steel tubing. This condition exposed employees to amputation hazards. (c)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not ensure that the 55-ton Edwards Ironworker, S/N: 29495501, was equipped with guards or otherwise protected from operator contact to the outgoing nip point angle shear portion located on the east side of machine. This condition exposed operators to crushing and amputation hazards. (d)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021 , the employer did not ensure that two Cincinnati Auto Shear Machines, Cincinnati Model: DB53 and Cincinnati Auto Shear Machine S/N: L191, and back to back Accurpress Brake Presses, where warning barrier chains were not connected while machines were in use. This condition exposed operators to struck by, crushing, and amputation hazards. (e)Dakota Bodies, LLC at 201 20th Avenue Southeast, Watertown, SD 57770: On or about and at times prior to December 21, 2021, the employer did not ensure that the Extensible Manufacturing Cell attached to a Mazak Optiplex 3015 III, S/N: 254819 with Extensible Manufacturing Cell 3015 S/N: 830189 anti-personnel fence had operational interlocks. This condition exposed operators to struck by, caught in, crushing, and amputation hazards.
