29 CFR  1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used:  (a) Dakota Bodies, LLC at 201 20th Avenue Southeast Watertown, SD 57201: On or about and at times prior to January 24, 2023, multiple areas of the plant, including designated common travel areas were exposed to arc flash due to welding curtains not surrounding welding operations.  No eye protection for exposure to arc flash was required or provided for non-welding exposed employees.  This condition exposed employees to temporary impairment of vision, temporary blindness and headaches associated with arc flash exposure.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)Dakota Bodies, LLC at 201 20th Avenue Southeast Watertown, SD 57201: On or about and at times prior to January 24, 2023, the employer did not ensure that the unused portion of the blade on the Hem saw, - Vertical Bandsaw  s/n: 1280918, was properly guarded with a guard or shield.  Approximately eight inches of saw blade was exposed while cutting steel tubing.  This condition exposed employees to amputation hazards.  Dakota Bodies, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number1569913, citation number 1, item number 4 and was affirmed as a final order on June 7, 2022, with respect to a workplace located at 201 20th Avenue Southeast, Watertown, SD 57201.

29 CFR  1910.212(a)(3)(ii): Points of operation of machinery were not guarded or guards were not designed and constructed to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   (a)Dakota Bodies, LLC at 201 20th Avenue Southeast Watertown, SD 57201: On or about and at times prior to January 24, 2023, the employer did not ensure that the point of operation on the 320-Ton Accurpress CNC hydraulic press brake (model# 732016) was properly guarded to prevent employees from having any part of their body in the danger zone during specific machine operations. Light curtains were disabled for certain operations. This hazard exposed employees to crushing and amputation hazards.  (b)Dakota Bodies, LLC at 201 20th Avenue Southeast Watertown, SD 57201: On or about and at times prior to January 24, 2023, the employer did not ensure that the point of operation on the 280-Ton Accurpress CNC hydraulic press brake (model# 717612) was properly guarded to prevent employees from having any part of their body in the danger zone during machine operations. Chains limiting access to the rear of the machine had been removed. This hazard exposed employees to crushing and amputation hazards.  Dakota Bodies, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1569913, citation number 2, item number 1, and was affirmed as a final order on June 7, 2022, with respect to a workplace located at 201 20th Avenue Southeast, Watertown, SD 57201.  Dakota Bodies, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1493340, citation number 1, item number 2 and was affirmed as a final order on December 31, 2020, with respect to a workplace located at 201 20th Avenue Southeast, Watertown, SD 57201.
