29 CFR  1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a) Carranza Construction LLC, at the jobsite located at 3404 Creek view St., Lot 71, Canal Winchester, Ohio 43110: On and/or preceding December 22, 2021, employees were engaged in roofing activities to an existing two-story residential single family house. The employer did not train and instruct employees on the hazards and regulations required to avoid and eliminate unsafe conditions such as, fall hazards and eye hazards.

29 CFR  1926.451(c)(2)(v):Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied.  a) Carrazna Construction LLC, at the jobsite located at 3404 Creek view St., Lot 71, Canal Winchester, Ohio 43110: On and/or preceding December 22, 2021, employees were engaged in roofing activities to an existing two-story residential single family house. Employees were working from an elevated platform while operating the Genie GTH-1056 rough terrain forklift (model: GTH-1056; S/N: GTH1011-14648), the forklift that supported the platform was moved horizontally while the employees were elevated, thereby exposing the employees to a 18 foot fall hazard.

29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a) Carranza Construction LLC, at the jobsite located at 3404 Creek view St., Lot 71, Canal Winchester, Ohio 43110: On and/or preceding December 22, 2021, employees were engaged in roofing activities to an existing two-story residential single family house. The employer failed to implement and maintain a safety and health program to ensure that fall hazards were identified and corrected onsite.  The Carranza Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1467393, citation number1, item number 1-a and was affirmed as a final order on May 11, 2019, with respect to a workplace located at Lot AG86, 4304 Rishel Street, Grove City, OH 43123.

The employer did not ensure that fall protection was utilized by employees, thereby exposing employees to fall hazards up to 18 feet.
