29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about December 23, 2021, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure employees performing servicing and maintenance activities on the sawmill debarker, sawmill bandsaw, sawmill edger, sawmill dust collector, and other machines and equipment at the facility were protected from unexpected energizing, startup, or release of stored energy. Machines and equipment used throughout the facility are powered by electric, pneumatic, and hydraulic power.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a) On or about December 23, 2021, the employer did not ensure the chain and sprocket wheels used for wood transfer between the bandsaw and edger machine were enclosed (guarded). The chain and sprocket wheels were less than seven feet above the shop floor and within one foot of the edger operator. b) On or about December 23, 2021, the employer did not ensure the chain and sprocket wheels used for wood transfer between the debarker conveyor belt and band saw feed conveyor were enclosed (guarded). The chain and sprocket wheels were less than seven feet above the working area.
