29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about December 29, 2021, employees were exposed to a struck by hazard while performing roofing activities on a residential home located on Lot 27 Emerald Woods, Columbia Station, Ohio 44028.  Employees were working on upper areas of the roof with tools and materials that could have fallen on lower employees and the employer did not ensure that the lower employees were wearing protective helmets.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about December 29, 2021, employees were exposed to a fall of approximately 21 feet while performing roofing activities on a residential home located on Lot 27 Emerald Woods, Columbia Station, Ohio 44028.  Employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.

On or about December 29, 2021, employees were exposed to a fall of approximately 21 feet while performing roofing activities on a residential home located on Lot 27 Emerald Woods, Columbia Station, Ohio 44028.  Employees were using an extension ladder to access the roof and the employer did not ensure that it was set at a safe angle.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about December 29, 2021, employees were exposed to a struck by hazard while performing roofing activities on a residential home located on Lot 27 Emerald Woods, Columbia Station, Ohio 44028.  Employees were using air powered nail guns to install the materials and the employer did not ensure that they were wearing protective safety glasses.  C.R.H. Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.102 (a)(1), which was contained in OSHA inspection number 1411224, citation number 1,  item number 1, and was affirmed as a final order on 8/14/19, with respect to a workplace located at 5805 Timberlane, Hudson, Ohio 44236.
