29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a. On or about February 9, 2022, employees were exposed to machine hazards associated with moving parts during servicing and maintenance of the #8 Flexographic Printing Machine. The employer failed to implement energy control application steps as the machine energy was not isolated prior to performing the servicing work [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements including LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about January 4, 2022, in the Production Area, the employer did not ensure that the in-going nip point of the rollers on the printer section of the # 8 Flexographic Printing Machine was adequately guarded, which resulted in an amputation injury.   b. On or about January 4, 2022, in the Production Area, the employer did not ensure that the in-going nip point of the rollers on the printer section of the # 5 Flexographic Printing Machine was adequately guarded.

On or about February 11, 2022, on the #5 Flexographic Printing Press, the employer did not ensure that the gears on the equipment were guarded.

29 CFR  1910.219(f)(3):Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a. On or about February 11, 2022, the chain and sprocket on the sheeter portion of the #8 Flexographic Printing Machine was not  properly guarded.   b. On or about February 11, 2022, the chain and sprocket on the sheeter portion of the #5 Flexographic Printing Machine was not  properly guarded.

This subparagraph does not apply to keys or setscrews within gear or sprocket casings or other enclosures, nor to keys, setscrews, or oilcups in hubs of pulleys less than twenty (20) inches in diameter where they are within the plane of the rim of the pulley.  a.
