29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements: On or about December 8, 2021, at the Cactus drilling site; employees were exposed to fall hazards due to the employer not ensuring that each flight of stairs was equipped with a complete stair rail system in the following instances: a) Site Manger's trailer #MH-0717 stairway was missing the mid rails. b) Directional Driller's trailer #MH-0657 stairway was missing the mid rails. c) Night Site Manager's trailer #MH-0759 stairway was missing the mid rails. d) Latrine trailer #CH-080 stairway was missing the mid rails.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about December 8, 2021, at the Cactus drilling site; employees were exposed to electrical shock hazards due to electrical breaker boxes, strain reliefs, missing parts in the following instances: a) The electrical cable going into the breaker box on the Night Site Manager's Trailer #MH-0759 did not have a strain relief. b) The electrical cable going into the breaker box on the Directional Driller's Trailer #MH-0657 did not have a strain relief. TanMar Companies, LCC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1384736, citation number 02, item number 002c and was affirmed as a final order on August 30, 2019, with respect to a workplace located at Waler State Unit 4 Lease, Well 3wa, Cactus Drilling Rig #139, 16.74 Miles Se From Pecos, TX 79772.
