29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. On or about March 26, 2024, Ensign Rig #759, TanMar Companies, LLC equipment exposed employees to trip and fall hazards to due employer not ensuring that no opening in a stair rail system exceeded 19 inches in the following instances: a) Two open staircases in the front and back of Trailer MH0697. b) One open staircase in the back of Trailer MH0814. c) One open staircase in the front of Trailer CH13. TanMar Companies, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.29(f)(4), which was contained in OSHA inspection number1624253, citation number 1, item number 1 and was affirmed as a final order on December 12, 2022, with respect to a workplace located at Patterson Rig # 289 rig.

29 CFR 1910.305(c)(4): Faceplates for flush-mounted snap switches. Snap switches mounted in boxes shall have faceplate installed so as to completely cover the opening and seat against the finished surface. Ensign Rig #759: On or about March 26, 2024, inside Trailer #MH0339R, a snap switch located inside the trailer had a broken faceplate exposing live electrical parts. This exposed employees to electrical shock hazards.

29 CFR 1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations did not have an enclosure for the receptacle that was weatherproof when the receptacle was covered (attachment plug cap not inserted and receptacle covers closed: Ensign Rig #759: On or about March 26, 2024, outside Trailer #MH0339R, a weatherproof receptacle had a missing cover. This exposed employees to electrical shock hazards.
