29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  On December 28, 2021, and times prior thereto, at this establishment, employees engaged in plastics manufacturing activities and were exposed to hazardous heated material that  resulted in burns on the employee's  forearm and palm.

29 CFR  1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.   On or about January 05, 2022, and time prior thereto, in the Line 5 winding area employees were exposed to struck hazards when steel carrier with tubes were not frame to prevent stack collapse.

29 CFR  1910.305(b)(2)(i):All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.   On or about January  5, 2022 and times prior thereto, in department 2,  employees were exposed to electrical hazards in the workplace when a cover on the electric switch was not provide.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about January  5, 2022, in department 2,  the flexible cord to an industrial pedestal fan was not connected with a functional strain relief device exposing's employees to electrical hazards.  The Nan Ya Plastics Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1313678, citation number 2, item number 1 and was affirmed as a final order on 05-02-18, with respect to a workplace located at Warton Texas.

29 CFR  1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.   On or about January 05, 2022, and time prior thereto, employees utilized a TCM #49 industrial powered industrial truck without a right front light working.
