29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On or about January 12, 2022, employees were exposed to crushing/pinch point hazards while working on and around the Cincinnati Press Brake (Serial #46049). The point of operation created between the bottom die and top die was not properly guarded, in-that the light curtains installed were not functioning. (b) On or about January 12, 2022, employees were exposed to crushing/pinch point hazards while working on and around the Cincinnati Press Brake (Machine #2030). The point of operation created between the bottom die and top die was not properly guarded, in-that the light curtains installed were not functioning. (c) On or about January 12, 2022, employees were exposed to crushing/pinch point hazards while working on and around the Cincinnati Press Brake (Machine #2024). The point of operation created between the bottom die and top die was not properly guarded, in-that the light curtains installed were not functioning. (d) On or about January 12, 2022, employees were exposed to crushing/pinch point hazards while working on and around the Trumpf Press Brake (TrumaBend V170). The point of operation created between the bottom die and top die was not properly guarded, in-that the light curtains installed were not functioning.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a) On or about January 12, 2022, employees working in and around the small shot blast bay were exposed to an in going nip point, in-that the chain and sprocket located at the entrance of the small shot blast bay was not properly guarded.
