29 CFR 1910.147(c)(1):Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about January 13, 2022, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure employees performing servicing and maintenance activities on the bending machine, saws, grinders, and other machines and equipment at the facility were protected from unexpected energizing, startup, or release of stored energy. Machines and equipment used throughout the facility are powered by electric, pneumatic, and hydraulic power.

29 CFR 1910.333(a)(1):"Deenergized parts." Live parts to which an employee may be exposed shall be deenergized before the employee works on or near them, unless the employer can demonstrate that deenergizing introduces additional or increased hazards or is infeasible due to equipment design or operational limitations. Live parts that operate at less than 50 volts to ground need not be deenergized if there will be no increased exposure to electrical burns or to explosion due to electric arcs. On or about January 13, 2022, the employer did not ensure an employee deenergized the live 480volt electrical parts inside the electrical/control cabinet of the T-50 pipe bending machine, before working near the electrical parts inside the panel.
