29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about May 3, 2022, an employee working in the pipe fabrication department was exposed to unguarded moving parts on the flare machine at 140 Blaze Industrial Parkway, Berea, Ohio 44017; the employer did not ensure the employee was guarded from the crushing and amputation hazards by guarding the flare machine's jaws, ram, and point of operation with one or more methods of machine guarding. b) On or about October 3, 2022, employees working in the pipe fabrication department were exposed to unguarded moving parts on the blue press at 140 Blaze Industrial Parkway, Berea, Ohio 44017; the employer did not ensure that employees were guarded from a crushing and amputation hazards by, guarding the blue press's' ram, and point of operation with one or more methods of machine guarding.

29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) On or about May 3, 2022, an employee working in the pipe fabrication department was exposed to unguarded moving parts on the flare machine at 140 Blaze Industrial Parkway, Berea, Ohio 44017; the employer did not ensure the existing guarding was affixed to the machine in that it was hinged on the top portion and unsecured at the bottom.
