29 CFR 1910.147(c)(6)(i)(C):Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspection being inspected. On or about June 06, 2025, the employer did conduct periodic inspections of energy control procedures, which included a review between the inspector and each authorized employee regarding their specific responsibilities prior to employees performing servicing and maintenance tasks. These tasks included conducting inspections, cleaning and lubricating machine components, replacing worn or damaged parts, performing die changes, and conducting safety checks on the AIM CNC Tabletop Bender AFT 3DS16.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. On or about June 06, 2025, the employer did not provide adequate machine guarding for the AIM CNC Tabletop Bender AFT 3DS16. The two-hand swipe controls, designed to safeguard the CNC machine, were positioned at the operator's station approximately 28 inches from the sliding chuck assembly and the movements of the die assembly. The placement of the controls, combined with the production cycle time, allows operators to reach into the danger area while the machine is in operation, exposing employees to serious hazards such as lacerations, amputations, and other permanent injuries from the moving mechanical parts.
