29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.  At the residential re-roofing job site, on the 6:12 pitch roof at 601 Stonehill Place in Saint Augustine, Florida:  On or about February 3, 2022, the employer exposed four roofing employees to 8-foot to 10-foot fall hazards from the roof eaves in that they were not protected by a fall protection system.

29 CFR  1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  At the residential re-roofing job site, on the 6:12 pitch roof at 601 Stonehill Place in Saint Augustine, Florida:  On or about February 3, 2022, the employer exposed four roofing employees to a 9-foot fall hazard from the top of the roof access ladder in that the ladder's side rails did not extend at least 3 feet above the roof eave.

29 CFR  1926.1053(b)(5)(i):  Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder):   a.   At the residential re-roofing job site at 601 Stonehill Place in Saint Augustine, Florida, at the left side of the house:  On or about February 3, 2022, the employer exposed four roofing employees to a 9-foot fall hazard from the top of the roof access ladder in that the horizontal distance from the top support at the eave to the foot of the ladder was more than one-quarter of the working length of the ladder.
