When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Circumstances where retraining is required include, but are not limited to, situations where:  a: Employees installing roofing materials at elevations of six-foot or more above the ground are not being protected by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on September 24, 2024, when eight (8) employees installing roofing materials on a 5:12 pitched residential roof located at 53 Mascotte Place in Jacksonville, Florida 32259, were exposed to the hazard of a 6 to 11-foot fall, in that fall protection was not utilized by employees, while performing roofing work .  Quality Labor Source was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1723504, Citation Number 2, Item Number 1, and was affirmed as a Final Order on July 9, 2024, with respect to a workplace located at 5923 Cedar Hills Boulevard in Jacksonville, Florida.  Quality Labor Source was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1576370 Citation Number 1, Item Number 1, and was affirmed as a Final Order on July 14, 2022, with respect to a workplace located at 601 Stonehill Place in Saint Augustine, Florida.
