At 11956 Diamond Springs Drive, Jacksonville, FL: On or about April 8, 2024, the employer failed to report a work-related in-patient hospitalization. The in-patient hospitalization occurred on April 2, 2024 and the employer notified OSHA on April 5, 2024.
