29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Troymill Wood Products, on or about April 6th. 2022, at the location of 17055 Kinsman Road, Middlefield, OH, 44062: Employees were performing a maintenance task below a saw blade without utilizing appropriate energy control procedures. The machine and saw blade were not locked out at the time, exposing the employee to amputation hazards.

29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Troymill Wood Products, on or about April 6th. 2022, at the location of 17055 Kinsman Road, Middlefield, OH, 44062: An employee was assisting in a maintenance task on the brewer line without the benefit of authorized employee training. The employee was utilizing an allen wrench with his head directly under a circular saw blade. The machine and saw blade were not locked out at the time, exposing the employee to amputation hazards.
