The employer learned of a confirmed COVID-19 case on 12/22/2021 and did not record it on the log until 01/25/2022. b) Montefiore Medical Center Hutch Ambulatory Center 1250 Waters Place Bronx, NY 10461: The employer did not record information on the COVID-19 Log within 24hrs. 29 CFR 1910.502(q)(3)(iii): The employer did not provide a version of the COVID-19 log that only includes, for each employee in the COVID-19 log, the location where the employee worked, the last day that the employee was at the workplace before removal, the date of that employee's positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms, if any were experienced, to all of the following: any employees, their personal representatives, and their authorized representatives by the end of the next business day after a request. a) Montefiore Medical Center Hutch Ambulatory Center 1250 Waters Place Bronx, NY 10461: The employer did not provide, for examination and copying the COVID-19 log to the authorized representatives by the end of the next business day after a request. Authorized representative request the COVID-19 Log on or about 01/07/2022 and was not provided the COVID-19 Log until on or about 01/25/2022. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE INSPECTION CASEFILE, THE EMPLOYER NEED NOT SUBMIT. CERTIFICATION.OR DOCUMENTATION OF ABATEMENT FOR THIS VIOLATION AS NORMALLY REQUIRED BY CFR 1903.19.
