Authorized Representative requested the COVID-19 log on or about 01/07/2022 and was not provided the COVID-19 log until on or about 1/25/2022. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE INSPECTION CASEFILE, THE EMPLOYER NEED NOT SUBMIT.
