29 CFR 1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a. On or about April 12, 2022, in the graphite milling room, an employee was exposed to amputation and/or crushing hazard when the chain and sprocket was unguarded on the Rotary Air Lock ROV 11893 which had been installed on storage tank T-222.
