29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Exterior of Building, (Rear of Structure): Employees engaged in interior framing activities were exposed to fall hazards of approximately 15ft  while using a portable aluminum extension ladder for access whose ladder side rails were not raised 3ft above the upper landing surface.  Condition noted on or about 2/28/2022.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.     a) Exterior of Building - Back of House: The employer did not ensure that the operator of the all terrain forklift was certified, on or about 02/28/2022.  WSZ Framing, LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.602(d): which was contained in OSHA Inspection Number 1272874, Citation # 1, Item # 4, issued on 04/17/2018 with respect to a workplace located at 111 Virginia Ave, Clifton NJ 07012. This citation became a final order on 5/07/2018.

29 CFR  1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:   a) Exterior of Building, (Rear of Structure): An employee engaged in roofing activities on a 6 on 12 pitched roof was exposed to fall hazards of approximately 18-20ft to the ground below as a means of fall protection was not utilized.  Condition noted on or about 2/28/2022.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks.  a- Building Exterior (Rear): A JLG- Lull Front end loader was used to raise an employee on a wooden pallet and left unattended with the hoisting boom fully raised as the outriggers remained unsecured above the second floor with a load of roofing materials. Condition observed on or about 02/28/2022.
