29 CFR  1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  (a)  On or about February 22, 2022, at an establishment located at 1097 Ehlers Road, in Neenah, Wisconsin; the employer did not ensure the Blower Application Co. Equipment No. 3B, Serial No 170952A3 which was in line with the Kugler-Womaki, Machine No. 00303; Machine Type:  HS542W was de-energized using an energy isolating device.  The employer relied on the Square D 2510 MCG3 Manual Starter push button to start/stop the blower when performing cleaning activities exposing employees to inadvertent or restart of the equipment.  (b) On or about February 22, 2022, at an establishment located at 1097 Ehlers Road, in Neenah, Wisconsin; the employer did not ensure the Blower Application Co. Equipment No. 3B, Serial No 170952A1 which was in line with the Belomatik 4666 was de-energized using an energy isolating device.  The employer relied on the Square D 2510 MCG3 Manual Starter push button to start/stop the blower when performing cleaning activities exposing employees to inadvertent or restart of the equipment.

29 CFR  1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about February 24, 2022, at an establishment located at 1097 Ehlers Road, in Neenah, Wisconsin;  employees were exposed to machine hazards associated with moving parts when they engaged in unjamming activities on the Blower Application Co. Equipment No. 3B, Serial No 170952A3 which was in line with the Kugler-Womaki, Machine No. 00303; Machine Type:  HS542W.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to unjam.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

On or about February 22, 2022, at an establishment located at 1097 Ehlers Road, in Neenah, Wisconsin;  employees considered affected employees under the control of hazardous energy were not trained in lockout/tagout to understand the purpose and use of the procedure and recognize when the energy control procedure is to be used.

29 CFR  1910.147(c)(4)(ii):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following  29 CFR 1910.147(c)(4)(ii)[A], (c)(4)(ii)[B], (c)(4)(ii)[C], and (c)(4)(ii)[D]:  (a)  On or about February 22, 2022, at an establishment located at 1097 Ehlers Road, in Neenah, Wisconsin; the Kugler Lock Out Tag Out, Rev 3, 7/17/2018 energy control procedure did not specifically outline the scope, purpose, authorization, rules, and specific procedural techniques for authorized employees to follow when performing servicing or maintenance activities on the Blower Application Co. Equipment No. 3B, Serial No 170952A3 and the Kugler-Womaki, Machine No. 00303; Machine Type:  HS542W.  (b)  On or about February 22, 2022, at an establishment located at 1097 Ehlers Road, in Neenah, Wisconsin; the Bielomatik Lock Out Tag Out, Rev 3, 3/3/2022 energy control procedure did not specifically outline the scope, purpose, authorization, rules, and specific procedural techniques for authorized employees to follow when performing servicing or maintenance activities on the Blower Application Co. Equipment No. 3B, Serial No 170952A1 and the Belomatik 4666.  The Dixon Ticonderoga Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1462163, citation number 1, item number 1 and was affirmed as a final order on April 8, 2020, with respect to a workplace located at 2525 North Casaloma Drive, in Appleton, Wisconsin.
