29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Lot 19 Greenhouse Rd., Holyoke, MA: On or about March 7, 2022, 2 employees were observed working from a raised aerial lift without being attached to the boom or basket. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1). Sage Engineering and Contracting, Inc. was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.453 (b)(2)(v) which was contained in OSHA inspection number1336488, citation number 1, item number 2b and affirmed as a final order on 2/15/2019 with respect to a workplace located at 339 State St., Springfield, MA.
