The written program shall be available for inspection by employees and their authorized representatives.   In March 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the Paint Area; entry into the Blue-Surf Oven was conducted to remove the sheet of metal on the bottom of the elevated oven floor and remove small parts which had fallen from the conveyor line.  The gas line was not properly isolated prior to entry.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about March 8, 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the North Core Room, employees were exposed to machine hazards associated with moving parts when they engaged in setup activities on the L20.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to unbolt the blow head and gas plate and top vertical rod.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR  1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about March 9, 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the South Core Room, the back side of the N16 was opening allowing access to unguarded moving parts exposing employees to struck by and/or caught between hazards.  The fence installed did not completely enclose the back to prevent entry.

(a)  On or about March 9, 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the North Core Room, the Laempe Reich L20 had exposed pinch points not guarded by the safety mat when the top plate descended contacting the horizontal part of the box.  (b)  On or about March 4, 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the North Core Room, the D75 was not guarded to prevent contact with the dies coming together exposing employees to caught between hazards.

(a)  On or about June 28, 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the North Core Room, on the backside of the N16, the mac valves used to control the clamps had three electrical junction boxes which were not effectively closed exposing employees to electrical shock hazards.  (b)  On or about June 28, 2022, at an establishment located at N2467 Vaughan Road, in Waupaca, Wisconsin; in the North Core Room, on the backside of the N16, the junction box used to adjust the gas pressure was not effectively closed exposing employees to electrical shock hazards.
