The excavation was approximately 5 feet 6 inches deep with walls not adequately sloped and not properly protected from cave-ins. Employees had not been trained in soil classification nor how select cave-in protection systems based on soil classification and conditions. ABATEMENT CERTIFICATION REQUIRED

The excavation was approximately 5 feet 6 inches deep with walls not adequately sloped and not properly protected from cave-ins. The soil and excavation were not inspected by a competent person to evaluate soil conditions and proper protective measures. NO ABATEMENT CERTIFICATION REQUIRED
