29 CFR  1926.451(c)(1)(ii):Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less thereafter for scaffolds greater than 3 feet (0.91m) wide.  On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601  a) Employees engaged in erecting/dismantling scaffold activities were utilizing scaffold  that was assembled approximately 40 feet without being tied into building to gain access roof . The employees were exposed to fall hazards of approximately 40ft to the ground below. Condition observed on or about 03/24/2022.

29 CFR  1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:   On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601  a) Employees engaged in erecting/dismantling scaffold activities were utilizing scaffold  that was assembled approximately 40 feet without being supported by base plates, and mudsills to gain access roof . The employees were exposed to fall hazards of approximately 40ft to the ground below. Condition observed on or about 03/24/2022.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.      On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601  a) Employees engaged in erecting/dismantling scaffold activities was accessing and egressing the third level scaffold platform that was bridged by (2) planks that crossed over approximately 4ft section to the wall. The employees were exposed to fall hazards of approximately 30ft to the ground below. Condition observed on or about 03/24/2022.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601  a) (Unprotected planked access) employees engaged in erecting/dismantling scaffold activities climbed onto leading edge wall to access (2) planked bridged that crossed over approximately 4ft section to third level scaffold platform. The employees were exposed to fall hazards of approximately 30ft to the ground below as a means of fall protection had not been utilized. Condition observed on or about 03/24/2022.
