29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Grief, Inc. on or about March 25, 2022, at the location of 975 Glenn Street, Van Wert, OH, 45891: The employer failed to conduct a periodic inspection of energy control procedures associated with the Bottom Crimper line of the LokRim department to ensure that the procedure is implemented properly and to ensure that the procedure is adequate to provide effective protection to the authorized employee during servicing and maintenance operations.

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Grief, Inc. on or about March 25, 2022, at the location of 975 Glenn Street, Van Wert, OH, 45891: The employer failed to ensure employees were protected while performing troubleshooting or adjusting activities at the Bottom Crimper line of the Lok Rim Department. The employer did not ensure that the energy control application steps to control the electrical, hydraulic, pneumatic, mechanical and gravitational sources were implemented prior to troubleshooting activities at the Bottom Crimper line resulting in crushing injuries and hospitalization of a maintenance employee.
