29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Pyramid Roller plate roller lacked effective guarding where employees feed the roller to put curves in sheet metal. This condition exposed employees to ingoing nip points, amputation, and crushing hazards.  (b) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Pedrazzoli horizontal bandsaw did not have the nonworking portion of the blade guarded. This condition exposed employees to amputation and laceration hazards.  (c) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Metal Muncher ironworker machine lacked effective guarding at the notcher and the punch. This condition exposed employees to amputation and crushing hazards.  (d) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Baileigh pipe roller lacked guarding on the exposed rollers. This condition exposed employees to ingoing nip point hazards and amputation hazards.  (e) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Hyd-Mech M20A horizontal bandsaw in the North Saw Shed lacked guarding over the nonworking portion of the blade. This condition exposed employees to laceration and amputation hazards.  (f) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Hyd-Mech M20A horizontal bandsaw in the 80 foot bay lacked guarding over the nonworking portion of the blade. This condition exposed employees to laceration and amputation hazards.  Abatement Note: Abatement certification is required for this item. 29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a) Midwest Fabrication & Steel, Inc.: On or before 4/5/22 the employer did not protect employees from injury in that the Accupress press brake was used without adequate guarding or distance as required by this standard. Employees bend parts with their hands within 4 inches of the dies. This condition exposed employees to caught-in, crushing, and amputation hazards.  Abatement Note: Abatement certification and documentation are required for this item. See enclosed "Certification of Corrective Actions Worksheet."  Midwest Fabrication & Steel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1285576, citation number 1, item number 3, and was affirmed as a final order on 3/28/18, with respect to a workplace located at 1015 E. Northern Ave. Pueblo, CO 81006.
