29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the jobsite located at the corner of Roe Ave & Delmar, Lot 68 in Overland Park, KS 66224: Employees were exposed to fall hazards. Employees engaged in residential framing activities, walking the top plates, were exposed to fall hazards of approximately 8 feet from the working surface of the top plate to the first level of the residence and 19 feet from the working surface of the top plates to the interior ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at the corner of Roe Ave and Delmar, Lot 68 in Overland Park, KS 66224: Employees engaged in residential framing activities were exposed to fall hazards of approximately 10 feet from the unprotected edge of the upper floor within the residence to the ground below while accessing and exiting the upper level in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the structure.
