29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) 990 Broadway, Dunedin FL, observed on or about 04/07/2022, employees in the process of lathe work on the exterior of the building were exposed to a 21'4" fall hazard, in that, a guard rail system was not installed along the top work platform of the tubular welded walk-through style frame scaffolding. Scaffolding was erected on the balcony of the 2nd floor which was 15 foot in height ground to overhang, scaffolding was 1 section high, 6'4", 3'6" wide by 7 foot long.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) 990 Broadway, Dunedin FL, observed on or about 04/07/2022, employees receiving materials from the 2nd floor balcony on the west side of the building were exposed to a 14 foot fall hazard, in that an adequate temporary guard rail system was not installed. Guard rail system installed consisted of one rail installed approximately 36 inches in height.
