On or about April 7, 2022, and at times prior to, employees are exposed to slip and fall hazards in the carousel and hydraulic press areas where oil and water are leaking onto the walking working surfaces.

29 CFR 1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toe boards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset  must be used.  On April 13, 2022 Employees accessing the second floor storage area were exposed to fall hazards when the fencing  had a hole was not secured.
