29 CFR  1910.124(j)(3): Periodic inspections of all dipping and coating equipment, including covers, drains, overflow piping, and electrical and fire-extinguishing systems were not conducted, and deficiencies were not promptly corrected:  On or about August 29, 2022 and at times prior thereto and times thereafter, periodic inspections were not conducted to identify concerns including but not limited to cracks in the microfinish tank containment and separation from the concrete floor, exposing employees to chemical hazards.

29 CFR 1910.132(d)(1)(i): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment:  On or about August 29, 2023, and at times prior thereto, a workplace hazard assessment to identify personal protective equipment for all areas of the facility had not been completed. Employees were exposed to hazards associated with the use of chemicals such as, but not limited to, methyl ethyl ketone, trichloroethylene and isopropyl alcohol.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about August 29, 2022, and at times prior thereto, hazard communication training provided to employees was not effective. Employees were exposed to hazard associated with contact with hazardous chemicals.  a) Safety data sheets for the solution being used in the microfinish process were not available to employees during training.   b) The hazard communication training provided to employees was not specific to either the chemicals or group of chemicals in use in the process.
