On or about April 7, 2022, at the facility located at 325 Tompkins St, Fond du Lac, WI, the employer failed to provide a walking-working surface free of trip hazards from missing and broken tiles and uneven surfaces.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. On or about April 7, 2022, at the facility located at 325 Tompkins St, Fond du Lac, WI, the #7 Toyota electric forklift, Model: 8FBE18U, S/N: 15239, had damage to the front right and left tires while still in service exposing employees to struck by hazards if the forklift tipped over or lost the load it was carrying.
