29 CFR 1910.119(d)(3)(i)(B): The employer shall complete a compilation of written process safety information before conducting any process hazard analysis required by the standard. This process safety information shall include information pertaining to the equipment in the process including piping and instrument diagrams (P&ID's): On or about February 14, 2023, at the establishment located at 325 Tompkins Street Fond Du Lac, Wisconsin; the employer did not have a complete compilation of process safety information pertaining to the equipment in the process including piping and instrument diagrams. Deficiencies in the completeness of the ammonia refrigeration system (ARS) and its associated P&IDs included, but were not limited to, the following: a) Compressor Room East, the Vilter High Stage Compressor, model number VMC440, identified as "CP-1", was labeled "CP-1 Vilter 446" on P&ID R111-02_EAST ENGINE ROOM. b) Compressor Room East, the Vilter High Stage Compressor model number 4512XLB, identified as "CP-2", was equipped with a hand valve that was missing a tag. The hand valve was labeled "CP-2 HV009" on P&ID R111-02_EAST ENGINE ROOM. c) Compressor Room East, the Vilter High Stage Compressor model number 4512XL, identified as "CP-3", was equipped with a pressure indicator labeled "CP-3 PI 001" that was not labeled on P&ID R111-03_EAST ENGINE ROOM. d) Compressor Room East, the Frick High Stage Compressor model number RXF50H, identified as "CP-4", was equipped with a hand valve labeled as "CP-4 HV003" that was identified as "CP-5 HV 003" on P&ID R111-03_EAST ENGINE ROOM. e) South-east Silo Room, the horizontal tank identified as "TK-5101", was no longer in service and was not removed from P&ID R111-08_SOUTH-EAST SILO ROOM. f) South-east Silo Room, the horizontal tank identified as "TK-5102", was no longer in service and was not removed from P&ID R111-08_SOUTH-EAST SILO ROOM.

29 CFR 1910.119(d)(3)(ii): The employer shall document that equipment complies with recognized and generally accepted good engineering practices: On or about February 14, 2023, at the establishment located at 325 Tompkins Street Fond Du Lac, Wisconsin; the employer failed to document compliance with the employer's chosen RAGAGEP including 1991 IIAR Bulletin 114, "Identification of Ammonia Refrigeration piping and system components" when Plate and Frame Heat Exchanger Surge Drum labeled as "V-34" was identified "V-32" on P&ID R111-20_GC-4 AREA. This hazardous condition exposed employees to inhalation hazards and burns from anhydrous ammonia liquid and vapors.
