29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycles:  Production Area, Wire Fabrication Room:  On or about April 1, 2022, the employer did not ensure that that point of operation on the Accupress CNC Hydraulic Press Brake (MN 7606, SN 12043 )  was adequately guarded to prevent employees from accessing the area during machine cycles.  This exposed employees to amputation and crushing hazards during normal operations of the equipment.  Abatement documentation is required for this item in accordance with 29 CFR 1904.19(d).
