When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided:   (a) At the Sheet Rock Lamination Machine line, located at 113 Milledgeville Road, Eatonton, GA: On or about April 20, 2022, employees performing warehouse operations, were exposed to slip hazards, in that the employer did not remove the glue leaking from the Glue Applicator onto the walking and working surface.

(a) At the Lamination Facility located at 113 Milledgeville Road, Eatonton, GA: On or about April 20, 2022, all employees working in the lamination warehouse were exposed to a potential smoke inhalation and fire hazard when the exit on the side of the building was blocked by equipment.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  a). In the maintenance department located at 113 Milledgeville Road, Eatonton, GA: On or about April 20, 2022, authorized employees engaged in service and or maintenance on powered industrial trucks (PIT) with 24 volts, gravity, and hydraulic energy sources were exposed to a crush- by hazard in that the employer did not develop specific written energy control procedures.   b). In the Lamination Facility located at 113 Milledgeville Road, Eatonton, GA: On or about April 20, 2022, authorized employees engaged in service and or maintenance on a conveyor with 480 volts and 120 psi energy sources were exposed to an amputation and/or a pinch point hazard in that the employer did not develop specific written energy control procedures.  c). In the Lamination Facility located at 113 Milledgeville Road, Eatonton, GA: On or about April 20, 2022, authorized employees engaged in service and or maintenance on glue applicator with 480 volts, 120 psi, and mechanical energy sources were exposed to an amputation and/or a pinch point hazard in that the employer did not develop specific written energy control procedures.
