A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) At the jobsite, employees working in the excavation were not provided with a ladder every 25 feet, exposing the employees to the hazards of cave-ins.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) At the jobsite, the employer did not ensure that the competent person in charge on site conducted an inspection of the excavation before allowing employees to enter it, exposing the employees to the hazard of a cave-in.
